Patient Forms
At Bluegrass Surgery and Laser Center, your time is as valuable to us as it is to you. Upon scheduling your surgery, a member of our staff will provide you with the required forms to fill out. We encourage you to take time to complete these before arriving on day of surgery, so that we may care for you promptly.
Insurance and Billing
Bluegrass Surgery and Laser Center accepts most major insurance. We will bill your insurance carrier for you; however, we cannot tell you what your policy will cover. Since your insurance policy and coverage are between you and your carrier, we urge you to contact them prior to your procedure.

I've had cataract surgery and both eyes now and the second one with Bluegrass was so much better. I highly recommend them as they are very professional and very caring.

Awesome service! From the check-in, through the surgery, to check-out. Staff is very courteous and informative, and made the procedure go smoothly. 5-stars service!

If you are a self-pay patient at Bluegrass Surgery and Laser Center, you should think otherwise. I have been dealing with them submitting my procedures to my insurance and thereby double-billing me three times now. Each time I'm told they'll look into it and reverse the claim, but I've gotten a new EOB three times now for the procedure I've already paid out of pocket for now. This is quite frustraiting.

From the time I arrived for my surgery until the time I left, everyone was so nice and professional. My surgery went wonderful and I could not be more pleased with this surgery center.

Everyone was very nice. Facility was clean and comfortable. My only complaint was not being seen until an hour after the appointment time. Too much time to be nervous.
